Pursuant to Art. 13 of EU Regulation 2016/679 ("Regulation"), UPMC Italy with registered offices in Palermo, Via Discesa dei Giudici 4 ("UPMCI") in its capacity of data controller, informs you that the personal data (also sensitive data) you provided filling out the "Request " and "On-line Contact" forms on the www.upmcvillamaria.it website of UPMC Hillman Cancer Center at Villa Maria ("Center") will be used exclusively for the purpose of processing your requests. The legal basis of the processing for this purpose is your request (Art. 6.1.b - processing is necessary for the performance of a contract to which the data subject is party or in order to take steps at the request of the data subject prior to entering into a contract; Art. 9.2.h - processing is necessary for the purposes of medical diagnosis, the provision of health or social care or treatment). Data conferment is optional. However, not filling out the fields labeled as "required" may affect the provision of the service. Data will be stored only for the time necessary to process your request.
If you give your consent, by ticking the box at the end of this form, your data (i.e., e-mail address and telephone number) will also be used to send you periodic information on the Center or other promotional material on UPMCI's activities. The legal basis of the processing for this purpose is your consent (Art. 6.1.a of the Regulation). Failure to provide consent will only entail the impossibility to receive information communications, but will in no way affect the possibility to submit forms and receive our feedback to your request. For this reason, your data will be stored for 24 months after their collection. You may withdraw your consent at any time, exercising your right as follows.
Data will be processed using paper and computer tools in order to guarantee their confidentiality and security. Your personal data will be processed by UPMCI employees and collaborators, acting in force of specific instructions on purposes and methods of data processing. Furthermore, your personal data could be disclosed for the purposes stated above to third parties, appointed data processors, providing ancillary or instrumental services to the data controller, or to independent data controllers, for the purpose of processing your request or to comply with the law. An updated list of all appointed parties can be requested to the Data Protection Officer or the data controller, at the addresses listed below.
In your quality of data subject, you also have the right to obtain from the data controller authorization to access, correct, delete, and to limit or deny the processing of your personal data (Art. 15 et seq., Regulation).The request must be filed to the UPMC Data Protection Officer (UPMC Italy DPO, Discesa dei Giudici 4, 90133, Palermo, Italy; e-mail: DPO@upmcvillamaria.it) or to the Data Controller (e-mail: email@example.com). A template of the request is available from the Italian Data Protection Authority website.
Should you deem that your personal data has been processed in breach of the Regulation, you have the right to file a complaint to the Italian Data Protection Authority (Art. 77, Regulation) or to start judicial proceedings (Art. 79, Regulation).
